Kids Choice Sports Policies and Waiver
Code of conduct policy:
Our goal is to provide a healthy, fun, and safe environment for everyone involved. We enforce a strict no-tolerance policy regarding the use of tobacco, drugs, alcohol, profanity, or any inappropriate or confrontational behavior during our programs. Kids Choice Sports, LLC reserves the right to suspend any registration. In the event a registration is suspended due to a violation of our code of conduct policy, no refunds will be given.
ON/OFF tab:
Occasionally, unforeseen circumstances such as gym availability or weather may result in late changes, postponements, or updates. Please check the on/off tab on our website, every week, within one hour of the start time time. Anytime something is postponed, or there's an update, you'll find it there.
Refunds:
Please note that once a season has begun, no refunds will be issued. If a player needs to withdraw before the start of the first session, they will receive a full refund. Kids Choice Sports, LLC reserves the right to cancel any session due to insufficient enrollment. Upon cancellation, a full refund would be made. Due to availability of gyms, and unpredictable weather conditions, gym and field space can be extremely limited at times. If a gym or field is not available on a scheduled day, we first try to make it up by adding a week on to the end of the season. When this is not possible, scheduled times, locations, and days, are subject to change. No refunds will be issued for any changes to the schedule. For any one day event, you may request to cancel your registration for a full refund, up to 72 hours before the date and time of the event. Cancellations between 25-72 hours will not be refunded, but may transferred to a friend. Cancellation requests made within 24 hours of the event date/time may not receive a refund nor a transfer. When you register for an event, you agree to these terms.
Waiver and Release of All Claims and Assumption of Risk:
I affirm that the participant ("Player") has my consent to participate in any program(s) offered by Kids Choice Sports, LLC. I acknowledge and understand that participation in these programs carries inherent risks of physical injury. I agree to assume full responsibility for any and all injuries, damages, or losses sustained by myself or my minor child, regardless of severity, as a result of participation. I understand that there is an element of risk inherent in youth sports, and as parent/legal guardian of the Player, I take full responsibility for his/her actions and physical condition. Furthermore, I understand that certain risks, dangers, and injuries due to things such as inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to youth sports programs exist. Therefore, it is recognized that it is impossible for Kids Choice Sports, LLC to guarantee absolute safety. I release and agree to indemnify and hold harmless Kids Choice Sports, LCC and its representatives, volunteers, agents, and any county, town, school district, or venue in which program is being held, from all claims or liability for loss, damages and/or injuries, to the named Player and/or any third parties person or property; caused by and resulting from any causes whatsoever.
Photo/Video Release:
I hereby give my consent for Kids Choice Sports, LLC to use any photos/videos of myself and/or minor child, in future Kids Choice Sports, LLC or School District brochures, flyers, websites, photos and videos of its programs and their participants, which may be used for promotional and instructional purposes. Furthermore, I understand that all such photos/videos remain the property of Kids Choice Sports, LLC.
Return to Play Waiver for Infectious Disease - COVID-19:
In consideration of being allowed to participate on behalf of Kids Choice Sports, LLC athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: Participation may expose individuals to infectious diseases, including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases, I HEREBY RELEASE AND HOLD HARMLESS Kids Choice Sports, LLC their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ AND FULLY UNDERSTAND THIS RELEASE OF LIABILITY and Assumption of Risk Agreement. I acknowledge that I am giving up substantial rights by signing it, and I do so freely and voluntarily, without any inducement.
